How To Edit Form 3921?
Shareholders and employees exercise options granted to them. When a shareholder exercises an option, Form 3921 is generated.
Note: the Form 3921 feature is only available for premium account holders. Kindly upgrade your account to unlock this feature. The company must file Form 3921 by the 31st of January.
Here are the steps to follow:
Step 1: Log into your Eqvista account and select the company account to generate Form 3921.
On the dashboard, click on “Compliance” on the left-hand side to get a drop-down menu.
Step 2: Here you will see all the reports available. Click on “Form 3921” to create a form for the options exercised. Once you do this, you will be redirected to the next page where you can begin the process of creating a form.

Step 3: Let’s assume you want to edit the company details on the form. To do this, click on “Edit” on the right-hand side as shown below.
By doing this, a new window will appear where you can change the address, state, country, ZIP code, and TIN as shown below.
Once you have done, click on “Save” button.
Step 4: Suppose you want to edit the shareholder address, just click on “Edit” button.
Note: You must already have a shareholder profile created. Check out the support article to add a shareholder.
Now you can edit the shareholder details such as address, city, ZIP code, state, and country.
Then, click “Submit” button to save the changes you’ve made.
Step 5: Similarly, you can edit the fair market value by clicking the “Edit” button.
To add a new fair market value, click on “Add Missing FMV” button and add details like “start date”, “end date” and “value”.
Note: There should be an FMV in every option’s exercised date in the selected year. If you add several FMV and FMV validity period overlaps, the system will take the most recent FMV.
After filling out the details, you can save the information by clicking on the “Save” button. Then, you will be redirected to the download page. Check out our support guide to understand the details of the downloaded form.
